About Us

Ebony Maynard-Moise APRN is dual board Certified Family Nurse Practitioner, specializing in Family Medicine known for her commitment to patient care and professionalism in every aspect of her clinical practice. Ebony has 13 + years operating in many areas of healthcare including, Neurology, Intensive Care, Home Health, Assisted Living, Long term care, Rehabilitation, Family Health, and Military institutions.

As a military veteran and former law enforcement officer, Ebony pursued a Master of Science in Nursing with a specialization in Family Medicine to combat the challenges her family has faced within the healthcare industry.

As an African American woman, and parent of a child with sickle cell disease, Ebony has became familiar with being overlooked by medical providers and the healthcare systems that lack knowledge surrounding caring for individuals with sickle cell disease. As a parent of a child suffering with sickle cell disease, in order to directly provide the best care for her son, Ebony pursued a career as a Family Nurse Practitioner allowing her to have a voice to actively manage his health care needs. With her knowledge, Ebony became committed to providing excellent healthcare to all individuals and to address health care disparities faced by the underserved populations of minorities and those combatting sickle cell disease. Ebony is tasked with advocating for those with sickle cell disease holistically including natural treatments which have helped her son prevent crisises. Ebony is passionate about primary care and knowledgeable in sickle cell disease through her direct personal and professional experience providing an integrative approach to the sickle community thereby improving patient healthcare outcomes.

She has established a reputation for delivering optimal patient care by improving health and survival in the diverse evolving landscape of healthcare. She remains deeply passionate about optimizing her patients well being and positively impacting the lives of patients and the communities she serves.
